RegistrationEarly registration until 30.6.2015. Registration of all participants is handled by the conference secretariat:
Please note that the on-line Registration Form is only accessible with a password. Simply fill in the Personal Data Form and a password will be sent to you. Upon receipt of the registration form and the payment, a Letter of Confirmation will be sent to the participants. Please present the Letter of Confirmation at the Registration desk at the Congress venue to pick up your materials. Early registration fees will only be confirmed if the registration form and the payment are received by the Congress Secretariat on June 30, 2015 at the latest. Registration forms received after October 7, 2015 will not be accepted as pre-registration. Please do not send any registration forms or payments after that date. You will have the possibility to register in the Congress venue. The on-site registration will be available from October 15, 2015. All participants will receive the Letter of Confirmation within three working days via e-mail or fax after the submission of their registration form. Registration fees

RegistrationOnly registered participants will be admitted to the scientific section, exhibition, poster section and courses. Registration of the pre-registered and new participants will be located on the ground floor of Nové Adalbertinum. Please check your congress material upon presentation. Late claims cannot be accepted. RefreshmentsComplimentary coffee and tea will be available in the exhibition area, lunches will be provided in the restaurant on the ground floor of Nové Adalbertinum. Both are free of charge for registered participants of the congress. Transport servicesTransport service to the congress venue and social programme will not be arranged. BadgesParticipants will receive a name badge. Everyone is requested to wear this badge during all congress activities. Cost of replacing a lost or mislaid badge: 4 EUR. Certificate of attendanceparticipants will receive a certificate of attendance at registration desk. Welcome drinkWelcome drink will take place in the exhibition area on the 2nd floor on October 15, 2015 and it’s free of charge for all registered participants. Tour of the water power station Hučák
The tour will be arranged on October 15, 2015 between 17:00 – 19:00 and it will last 45 min. The power station was bulit in Art Nouveau style. The number of visitors is limited. You can express your interest during the on line congress registration.
Social evening for the orthodontic assistants and techniciansParty will take place in the Level Music Club on October 15, 2015, Velké náměstí 166/1, Hradec Králové. Presidents receptionPresident´s Reception will take place in the Museum of Eastern Bohemia in Hradec Králové, Eliščino nábřeží 465/7 on October 16, 2015. Free admission for category 1, 2, 3, 4, 5, 9, 10. Exhibition areaExhibition will take place on the 2nd floor of Nové Adalbertinum. Entry is included in the registration fee. Cancellation and refundsThe Conference Secretariat must be notified in writing about the cancellation of registration by fax (+420) 284 001 448 or e-mail COS2015@guarant.cz . Telephone cancellations will not be accepted. The appropriate refunds will be made after the Congress. The following cancellation conditions apply:
Methods of PaymentRegistration fee must be paid in EUR by one of the following methods: 1. Credit cardFor payments through the Online Payment System (available after login) only MasterCard/Eurocard, Visa and Diners Club credit cards are accepted. These payments will be charged in EUR. For payments using American Express credit cards please complete Credit Card Payment Authorization Form and kindly send it signed by a card holder to the Congress Secretariat by fax (+420 284 001 448) not by e-mail for security reasons. The payment will be charged in local currency – Czech crowns (CZK) – at the exchange rate of the Czech National Bank valid on the date of payment. . The approximate exchange rate is 1 EUR = 27.755 CZK (February 2015). Credit Card Payment Authorization Form can be also used for MasterCard/Eurocard, Visa (will be charged in EUR) and Diners Club (will be charged in CZK). Bank transferPlease note that payment by bank transfer is possible by October 7, 2015. After this date only payments by credit cards will be accepted (you can either use Online Payment System or send us an Credit Card Payment Authorization Form ).
All payments made by bank transfer have to be net of all bank charges. The payer pays the bank charges of their bank, and the beneficiary pays the charges of their bank, if any. |
